Fresno HOPE (Health, Outreach, Prevention, Equity) seeks to identify specific health inequities and provide sustainable, long-term health outcomes to our vulnerable Fresno communities.

Who We Are

The Fresno HOPE Pathways Community HUB (Fresno HOPE PCH) is the first nationally certified HUB in California. Fresno HOPE PCH is a network of Care Coordination Agencies focused on synchronizing resources to mitigate health disparities in Fresno County.

Care Coordination Agencies hire local Community Health Workers (CHWs) to diminish linguistic and cultural barriers and improve health within their communities. The evidenced-based Pathways Community HUB InstituteSM Model encourages CHWs to perform services until completion and cultivate compelling data to identify and transform specific health barriers in the community.

There are 21 standardized pathways to aid Community Health Workers in alleviating risk factors to strengthen care coordination in the Fresno community. Risks are identified and addressed through a whole person, whole household approach to build a more resilient community.

The Pathways Community HUB model aligns with FCHIP’s vision and values of whole-person care that improves health, reduces costs, and promotes equity.

Through the advocacy and investment of the Fresno County Department of Public Health, the Fresno County Board of Supervisors approved a $12 million contract for FCHIP to implement the Fresno HOPE PCH.

Collaboration with Care Coordination Agencies started in 2020 with work on COVID-Equity Partners, working to address the COVID-19 pandemic that disproportionately affected our communities. As health inequity became more apparent, our local community organizations were at the forefront of providing health education, contact tracing, and social resources for at-risk populations.

OUR PARTNERS

Fresno HOPE is proud to have six Care Coordination Agencies with over 30 CHWs to serve under-resourced populations – largely medically underserved immigrants and refugees, African Americans, and individuals with disabilities.

Through the Pathways Community HUB model, we dismantle systemic inequities by empowering the community to be the driver of their health.

WHY COMMUNITY HEALTH WORKERS MATTER

Local Community Health Workers provide culturally and linguistically appropriate services that reflect our diverse population in Fresno County. Using the PCH model, CHWs offer families the opportunity to be decision-makers in their own health outcomes by guiding them through identifying risks and choosing the appropriate social, mental health, and medical services resources to remove barriers to health.
